Public Notes: PLEASE READ THIS INFORMATION CAREFULLY BEFORE REGISTERING Individuals who register for this event will receive the First Dose of Pfizer Covid-19 Vaccine.

**YOU MUST BE 12 YEARS OLD or OLDER TO REGISTER FOR THIS PFIZER COVID-19 VACCINE CLINIC** Minors Must Be Accompanied by a parent or guardian. Minors do not need to provide identification. 24 Hours after your First Dose visit has been entered and recorded, you will automatically be registered to receive the Second Dose of Pfizer Covid-19 Vaccine on September 8th, 2021. You will be given the same appointment time as the one you select for your First Dose Appointment.

Be sure your schedule allows you to come for your Second Dose on September 8th, 2021, BEFORE registering for this first dose clinic. You are not guaranteed a second dose of vaccine if you miss your appointment.

Be certain the email address you enter when you register is accurate. Entering an incorrect email address will prevent you from receiving confirmation of your First Dose appointment. Entering an incorrect email address will also prevent you from receiving confirmation your second dose appointment has been scheduled. Only individuals who are not ill are able to be vaccinated. If you have signs or symptoms of any illness please do not come to this location.

To expedite traffic flow through the clinic area, passengers are requested to ride in the back seat behind the driver. The vaccine will be administered through the open driver's side window.

Everyone in the vehicle must wear a mask when vaccines are being administered. No one is allowed to exit the vehicle while on clinic grounds.

Please wear clothing that makes it possible to easily inject vaccine into the left shoulder muscle. Please have your coat or jacket off.
